
If you are an expat looking to buy an insurance plan, there are many insurance providers that cater specifically to foreigners like you. But before you decide on a plan or provider, there are four important factors you need to consider:
- Type of plan: Most, if not all, insurance providers offer at least three to five types of plans, starting with a basic plan that only provides coverage for major medical/hospitalization services to a high-level plan with comprehensive coverage that includes routine health check-ups, maternity, and a generally better outpatient coverage. Some providers may even offer a “worry-free” plan that comes with unlimited hospitalization and outpatient services coverage.
- Exclusion: Exclusions are general policy terms that all insurers impose, and these can vary widely from insurer to insurer. It’s always worth reading the exclusions in the policy wording or (terms- & conditions) before making a decision. Note that even the highest premium plans have exclusions; and a high premium does not automatically mean it’s a better plan than a lower priced plan.
- Area of coverage: If you only need coverage within a certain region such as, for example, South East Asia or Africa, then it’s worth checking out plans specifically created for these particular regions because they are usually cheaper than plans that offer worldwide (excluding the U.S.) coverage. Note that most plans include worldwide emergency cover (including the U.S.) for temporary trips outside of one’s area of coverage.
- Pricing: When choosing a plan, you should consider community rating, a pricing system for health insurance that enables everyone to purchase the same product at the same price and renew the policy. Community rating is beneficial for consumers because they can be assured that their age, location, and other individual details won’t impact the product’s price and terms. On the other hand, if both community rating and performance rating apply to the plan’s price, it means that the rates will be based on your personal claims history too. Ask your advisor or the insurance provider directly about a plans renewal policy; what increase to be expected and how they increase your plan in case of a claim, before making any decision.
Here are a few companies that offer good health insurance options for expats.
- Cigna Healthcare – one of the most known expat insurance providers out there. They offer a variety of plans, from basic (Silver) to comprehensive (Gold) to worry-free plans (Platinum). However, Cigna is one of the most expensive providers as well.
- GeoBlue – is known for its coverage plans in the U.S. and is usually recommended for expats who need health coverage in the U.S. The company provides considerable access to the U.S. healthcare network.
- Passport Card – one of the most unique health insurance providers globally through their credit card member card, which provides a tool for easy & bureaucracy free direct claims handling without the need to go through any pay & claims process. Depending on the region / country, but also area of coverage their very comprehensive plans can be affordable, but usually are in the top price range, which however is in line with the service and benefits they provide. Passport Card’s plans are underwritten by MGEN and Allianz.
- Allianz Care – a true global brand with worldwide reach and customer service in multiple languages and tailor made available plans based on clients needs. Allianz price range is comparable with Cigna, which means you are paying for the name as well. If you want a solid and trustworthy insurance on your side, but money is secondary, then Allianz is your option.
- William Russell – offers several types of health insurance plans: basic hospitalization plan (Bronze), mid-level plan (Silver Lite), comprehensive plan (Silver), and an all-around plan (Gold) and depending on the region and area of coverage their plans can be very competitive. William Russell’s plans are underwritten by Allianz.
- Morgan Price – is an insurance provider based in the UK that offers great regional options. Their Flexible Choices Asia plan provides great deals with a variety of modules in multiple regions, such as South East Asia, Africa, or South America. Morgan Price plans are underwritten by Antea International Insurance.
- ACS – very competitive rates for anyone residing in none high cost countries and is looking for a budget plan, with a variety of different tailor options from just basic Inpatient plans (Bronze, Silver Gold, Platinum) to all-inclusive options including Outpatient, Wellness, Dental, additional Evacuation & Repatriation plan. ACS’s plans are underwritten by MGEN.
- IMG Global/IMG International – through their Global Prima plan (former ALC), IMG is offering a variety of affordable plans, from Basic Inpatient to top tier all-inclusive packages, which can be one of the lowest price options, depending on the region. IMG’s plans are underwritten by SiriusPoint.
- Genki – your Digital Nomad and Perpetual Traveller specialist. Genki has no Basic Inpatient only plan yet, but their plans are comprehensive, fair priced and truly for people on the road permanently with worldwide cover and no change of price if “moving” to another country. Their unique position is an instant online underwriting procedure with the possibility of covering pre-existing conditions with premium loading. Genki plans are underwritten by Squarelife
- BDAE – this Germany-based insurance provider offers valuable plans for reasonable premiums. All of its plans are offered in five specific regions and provide coverage only in certain zones within the insured person’s country of residence and the zones outlined below. For that reason, the plans’ prices are relatively lower compared to similar plans that include coverage in other countries. Bdae does not exclude pre-existing conditions; either an application is accepted with premium loading for pre-existing conditions, or declined entirely.
- April International – this well known France based insurer provides multiple plan options from a purely Emergency only plan up to their top tier “all-inclusive” Premium plan with a variety of plan adjustment options, which helps customers to tailor their plans based on their actual needs. For bigger families with more than 2 children, April in particular is a great option because they are free of charge.
- Expatriate Group – a smaller provider based in the UK with straight forward plan choices from Inpatient only to all inclusive comprehensive plan. Their plans are underwritten by UK based Strategic Insurance Services Limited.
- Vumi – is a good alternative to GeoBlue for U.S.-based expats, as it has the same features and coverage as GeoBlue.
- Now Health – these plans are quite comprehensive and come with high annual limits. Now Health has two product lines to offer, SimpleCare and WorldCare. SimpleCare is a budget plan with limited hospital access within the covered region. On the other hand, WorldCare is more comprehensive, has a worldwide coverage, and comes with several options: basic hospitalization coverage (Essential); including outpatient services included (Advance); including dental services (Excel); and a high premium plan that includes maternity coverage (Apex).
- HCI (Health Care international) – HCI group includes a couple of specialized brands such as IntegraGlobal, Marninesure or Ocean Care. For anyone looking for cruise or long-term boat trip medical coverage, then those are very good addresses with specialized customers service focusing on people living offshore. Their plans are underwritten by MGEN and VYV International Benefits

However, if you are rather looking for a budget option with good service instead, then you may have a look at the following alternative recommended solutions:
- Basic – Inpatient / Hospitalization only

- Maternity coverage – including comprehensive Maternity benefits
- USA option – including elective coverage in the US without duration limit

Alternatively, getting expat health insurance through a broker is smarter than going straight to an insurance company. Brokers give unbiased advice based on your needs, not a company’s sales goals. They compare plans across companies – something you will never get from an insurance company.

International Insurance Plans with U.S. Coverage
Most of the above options and other international medical insurance plans can be purchased with elective coverage in the US, although most plans provide emergency coverage if the insured is traveling to the U.S. temporarily, such as for a vacation or business trip.
However, there are a few plans that have particular advantages over others, and these are plans mostly from American-based providers that know the needs of U.S. citizens living abroad:
- Cigna: Elective U.S. coverage for U.S. citizens is considerably cheap, but it provides only three months coverage in the U.S. while living abroad. For any other nationalities, their U.S. coverage is as expensive as most other providers’.
- IMG US (Global Medical Insurance) : They offer worldwide coverage with additional co-pay depending on the hospital.
- Within U.S. PPO Network = no co-pay
- Outside US PPO Network = 20% co-pay for the first $5,000 cost; nil copay afterwards
- VUMI: There is no additional copay or deductibles for U.S. elective cover, but the overall annual limit for the U.S. is $3,000,000
- GeoBlue: This is good for U.S. citizens, in general.
Plan Explanation
Let’s take a look at the following terms in detail.
Annual Limit
Annual limit is the total maximum amount that an insurance provider will pay you in a particular year. Generally speaking, the higher the limit, the better.
Note that the annual limit can also depend on the country you are living in. If you are living in a country like Thailand or Indonesia, an annual limit of $1,000,000 is usually enough. If you are living in a developed country, you may need a higher limit than that, just to be safe.
Area of Coverage
Area of coverage refers to the country or countries where you will be covered by your health insurance provider. Many IPMI plans (international private medical insurances) come worldwide, excluding the United States coverage (including emergency cover in the US). However, there are also many options for regional cover such as South East Asia or Africa for example, which can be significantly cheaper.
Most insurance providers have an option to provide coverage in the United States, but the premium will be significantly increased.
Hospital Room
If you are hospitalized, your insurance provider should pay for the hospital. If the costs are covered in full or just partially depends on the policy. Some policies for example have room restrictions up to a certain amount per day.
Note that some providers may pay only for a semi-private room, while some providers may not pay for premium “5 Star” hospitals in certain countries.
Surgery Fees
If you need to have surgery, your insurance will cover the cost. There can be sublimits depending on the policy, but usually it is fully covered. Please note that some plans may not pay fully for the surgeon’s assistant fee or other related surgery costs (always check the detailed benefits; especially for hospitalization coverage).
Hospital Expenses
Hospital expenses refer to general expenses, such as nursing fees, medications, and so on.
ICU
ICU coverage usually includes both regular intensive care units and cardiac intensive care units.
Cancer Treatment
Cancer treatment coverage may not include preventative surgery. However, if a doctor recommends surgery because the cancer is early-stage, then the treatment will be covered under the cancer treatment benefit unless it is a preexisting condition.
Parental Accommodation
Some plans may give an additional daily budget for parents to stay with their children while hospitalized.
MRT, CT, and PET Scans
If you get hospitalized, all basic plans should cover the costs of MRI, CT scans, PET scans, and medical imaging, including X-rays.
On the other hand, basic plans might not cover such procedures for outpatient treatment costs.
Physiotherapy
If physiotherapy is deemed necessary, your insurance may pay for it. If provided under a basic hospitalization plan, then as long as treatments last in the hospital. For plans including Outpatient, physiotherapy is usually included up to a certain amount of sessions and / or limit.
Home Nursing
If you need home nursing, the insurance company may also pay for it. The company usually imposes a limit on how much they pay per day. And they may have a cap of 30 visits per year, on average.
Medical Evacuation & Repatriation
IPMI plans usually come with medical evacuation to the nearest appropriate medical facility, which can deal with one’s condition.
Such medically necessary evacuation will always be within the area of coverage. As soon as you are able to travel again, your insurer may arrange a transport or pay for the return to your country of residence.
Unlike for travel medical insurances, Repatriation under IPMI plans does not mean back to your home country. However, some providers offer additional “Home Country Evacuation & Repatriation” coverage.
Maternity cover & Newborn Care
Your insurance plan may pay for the cost of Maternity including pre- & postnatal treatments and the delivery. Usually there are particular designed plans for Maternity or such benefits are available as an add-on; every insurance provider has their own strategy for maternity related benefits.
Maternity benefits usually come with a waiting period of 10 to 12 months.
Newborn care is usually automatically included if maternity benefits are included in the plan, but the terms differ slightly between all providers. Generally speaking, the newborn is covered under the mothers maternity benefits for the first 30 days and can be enrolled into the mothers or the family policy without further underwriting within the first 30 days. If the newborn was not covered under the mothers policy, then usually the baby can only be enrolled after the first 30 days and underwriting is required.
Outpatient Treatment
Outpatient treatment means any minor medical issue which can be treated instantly without the need of admission to a hospital. However, many basic Inpatient only plans also cover Outpatient surgeries or Outpatient cancer treatments to avoid unnecessary high hospital charges.
Outpatient options vary significantly between different insurance providers, from low annual limits such as just $1,000 annually with many sub-limits for different treatment types, up to even unlimited Outpatient coverage without any limits.
Deductible / Excess
A deductible refers to the amount of money you need to pay out of pocket first, before the insurance company jumps in and covers your treatments. The higher the deductible / excess, the higher will be the saving on your insurance premium and usually it does make sense to work with a deductible / excess.
Excess is mostly used in the same context as a Deductible (per annum), but can also be used as Excess per condition and it is important to understand the difference and to check these terms before signing up.
Deductibles can be only for inpatient treatments, or just for Outpatient, or for all treatments in general; ever insurance company has their own terms with regards to Deductibles / Excess & co-pays.
Co-payment
Co-payment refers to the percentage you need to pay out of pocket when you make a claim. (Excess is sometimes used in this context as well, which is then a fixed amount per claim or condition)
For example, if your plan has a 10% co-pay and you are making a claim for a total of $10,000, you will have to pay $1,000 yourself and the insurance company will pay for the rest ($9,000).

If you’re over 65 years old, your options are more limited. Cigna Healthcare accept new applicants without an age limit. But it’s not the cheapest option.
